Home Care Services

Our Care Connect staff is able to assist our clients in their home environment, and dedicate themselves to assisting individuals with daily activities such as using the bathroom, preparing or eating meals, executing hygiene and grooming habits, medication reminders, bathing, and completing housework or other chores. 

Depending on the level of support provided, long-term care giving can take a toll on the caregiver’s physical and emotional health. Consider your loved one’s condition and your health and skills to determine whether in-home care is appropriate or if more specialized care is needed.
A caregiver cares for the health and well-being of someone who needs help with daily tasks and activities. Your aging loved one may need a caregiver because of an injury, mobility or memory issues, illnesses, or chronic conditions that make everyday chores more challenging.

Homemaking and Companionship Services

Homemakers and companions provide non-medical care to support  the well-being of a person in their home. Homemakers and companions provides support by helping prepare meals and assisting with routine tasks.

Companion care is designed to help combat loneliness, but also encourage older adults to remain social and engage in activities of daily living. For example, if your loved one is in the proper physical shape, their caregiver may encourage them to accompany them on a walk around the block or a stroll through the park and more.

Homemaker services provide assistance to persons with the inability to perform one or more of the following activities of daily living: preparing meals, shopping for personal items, managing money, using the telephone or doing  light housework  and more.

End of Life Services

End of life care services offer physical and emotional support for individuals with a terminal illness during their final stages of life. Caregivers provide assistance to your loved one while giving your family the support they need at this difficult time

Care giving at the end of life involves much more than the practical tasks of helping a person with terminal illness. It's also about letting your loved one know, through your words and actions, of your love and commitment. It's about promoting an atmosphere of peaceful acceptance. Bringing some relief to the families and loved ones.

End-of-life care is focused on maintaining quality of life while offering services to ensure comfort to the client, while providing the best care possible.  A major component of end-of-life care is the focus on allowing patients to pass with dignity while providing their loved ones the support they need.
